In the event Cyberonic takes corrective action due to a violation of the Cyberonic Terms of Service,
Cyberonic shall not refund any fees paid in advance of such corrective action.
Service requires use and purchase of authorized customer premises equipment.
Service is One-year term
starting the 1st day of the month. If required the first month of
service will be calculated as partial. This agreement will be
automatically renewed at the end of the initial term on a month-to-month
basis.
Taxes, surcharges, universal service fees and other mandated charges are not included in the quoted
prices and will be added to the customer’s bill.
Payment is due on the
1st day of every month and will be billed 15 days in advance to a valid
credit card (MasterCard, Visa, American Express or Discover). For your
convenience four months billing is available. Payment is due in advance.
A $25 late fee will be applied to any overdue invoices.
Actual transmission
speeds may vary depending on the distance of the installation from the
nearest Network Node or based on the quality of wireless signals.
This service is for IP data communications only. No local telephone, including emergency, service is
provided.
Installation is dependent on availability in the customer’s geographic region, and on availability of
adequate facilities.
Professional
Installations are performed Monday through Friday between 8AM and 5PM
for additional charge of $250.00 if required. Appointments for
installation are made in AM or PM timeframe. Barring unusual
circumstances, installations should take about 2 hours.
Professional
Installation appointments may be rescheduled up until 12 PM (Noon) of
the previous business day. There will be no charge to reschedule the
original appointment if notice is received before 12 PM (Noon) of the
previous business day. Subsequent rescheduling requests will result in a
$15 fee.
Requests to reschedule after noon of the previous day will result in a $250 charge.

One time installation and activation is non refundable after the initial 10 day trial period. Thirty days
cancellation notice in writing or telephone is required and in return a cancellation number will be
issued from Cyberonic. Please keep the cancellation confirmation number for your record.
If the customer requests a service technician to be dispatched and no fault is found with the circuit, a
charge will be incurred.
In the event of early cancellation of this Agreement prior to the end of the term, early termination will result a $250 fee.
The Wireless equipment we will provide is 1) a router or a modem, 2) wireless antenna, 3) data card.
Customer is responsible for configuring and provide all software required to establish network connectivity.
